Patients often ask which is the best silicone breast implant. There is no universal answer. A natural and proportionate result depends less on choosing the largest size and more on matching implant dimensions and behaviour to the chest, breast envelope, skin quality and desired look.

Silicone gel, saline and the implant shell

All breast implants have an outer silicone elastomer shell. They may be filled with silicone gel or saline. Silicone gels vary in cohesivity, which influences how the implant holds its shape and feels. Saline implants are filled with sterile salt water and behave differently if they rupture. Availability and approved indications differ between countries.

Implant surfaces may be smooth or textured. Surface choice has safety implications. The FDA states that breast implant-associated anaplastic large cell lymphoma, or BIA-ALCL, occurs more often with textured implants than smooth implants. Your surgeon should explain the specific device, its evidence, regulatory status and known risks.

Round, anatomical and ergonomic behaviour

Round implants can create fullness in the upper breast, although the final appearance also depends on gel behaviour, placement and existing tissue. Anatomical or teardrop implants are designed with more volume toward the lower pole and may suit selected reconstructions or aesthetic goals, but rotation can alter their shape.

Some modern round implants use gels designed to redistribute with posture and are marketed as offering ergonomic movement. These descriptions do not guarantee a natural result. The surgeon must match the device to the patient rather than match the patient to a popular implant.

Implant size is more than cubic centimetres

Volume is usually described in cubic centimetres, or cc, but two implants with the same volume can have different widths and projections. Chest width, existing breast base, tissue thickness and the amount of stretch that the skin can safely accept are crucial.

Oversizing can increase tension on tissues and may contribute to visible edges, rippling, bottoming out, stretch marks or earlier revision. An implant that is too narrow, too wide or poorly positioned may look less harmonious even when the volume sounds appropriate.

What does implant profile mean?

Profile describes how far an implant projects forward relative to its base width. Lower profiles are broader with less forward projection. Moderate profiles balance width and projection. Higher profiles are narrower for a given volume and provide more projection. Profile alone does not determine cleavage or upper-pole fullness; chest anatomy, implant position and soft-tissue coverage remain important.

How we choose an implant for each patient

Planning begins with the result the patient wants: subtle restoration, upper-pole fullness, more projection, correction of asymmetry or reconstruction after weight change or pregnancy. We then assess breast and chest measurements, nipple position, skin quality, existing tissue, asymmetry and activity. Sizers, diagrams or three-dimensional simulation may support the discussion but cannot predict healing perfectly.

The plan may involve placement above or below the muscle, a dual-plane approach, fat transfer, a breast lift or no implant at all. When the skin envelope is loose, increasing implant size is not always a substitute for a lift.

Safety and long-term follow-up

Breast implants are not lifetime devices. Complications can include capsular contracture, rupture, displacement, asymmetry, pain, infection, changes in sensation and the need for revision. The FDA recommends structured informed consent and provides a patient decision checklist. Patients with silicone implants should also discuss the recommended imaging schedule for silent rupture and keep their device details.

The best implant is therefore the one chosen after an individual examination and an honest discussion of proportions, trade-offs and long-term care—not the implant that is currently most advertised.
